Our team will attempt to begin the scheduling process within 24-48 hours of receiving a referral. 

If you are referring from a doctor's office, we will coordinate with a local DME supply company to schedule the appointment.

Insurance Information

We accept many major medical insurance plans. Please inquire if the patient has a less common insurance.

For patients with unique insurance circumstances, we offer a self-pay rate. Please contact us for more information.

What to Include on Referrals

We require a signed OT Rx in order to schedule a patient. We have attached our referral form below to utilize, or you may use your own. 

Please include the patient's insurance policy on the referral and all demographics.

Are You an ATP/Supplier?

If you are an ATP or supplier looking to schedule an appointment for a referred patient, please email us the referral/insurance, information/demographics, and your availability.

Depending on your insurance and the type of appointment needed, additional availability may not be reflected on the calendar. If you require a specific date or time, please reach out to us and we will do our best to accommodate your needs.

    LimbTech operates on Eastern Time (ET). The scheduling calendar allows you to change the displayed time zone to match your location, so please double-check the selected time zone when reviewing available appointments.
